  • Lead (Transformation & Digitalisation) to be based at C-Care

  • selected candidate will be responsible for leading the end-to-end delivery of
  • assigned Group transformation and digitalisation projects across C-Care
  • regions, clusters and corporate functions. The role will drive current-state
  • assessments, process redesign, requirements definition, implementation,
  • adoption and benefits realisation, ensuring that initiatives are delivered
  • within scope, timelines and governance frameworks. Working closely with project
  • sponsors, operational leaders, multidisciplinary teams and external partners,
  • the incumbent will facilitate sustainable business improvements and digital
  • transformation initiatives that support C-Care’s strategic objectives and

deliver measurable organisational impact.

What you will be doing:

  • Lead assigned transformation and digitalisation projects from initiation
  • to formal closure, including project charter development, scope definition,
  • objectives, deliverables, scheduling, budget monitoring, resource allocation,
  • dependency management, implementation, operational handover and project

closure.

  • Translate strategic priorities into actionable project plans,
  • workstreams, milestones, responsibilities, governance structures and success
  • metrics, ensuring delivery within agreed timelines, scope and quality

standards.

  • Coordinate cross-functional teams, project owners, subject matter
  • experts and external vendors, driving follow-up actions, resolving bottlenecks

and escalating key decisions or constraints when required.

  • Plan and coordinate user acceptance testing (UAT), go-live readiness,
  • transition activities, stabilisation support, operational handover and

post-implementation reviews.

  • Conduct detailed assessments of existing (AS-IS) processes with
  • frontline and support teams to identify pain points, root causes, bottlenecks,

control gaps, risks and improvement opportunities.

  • Facilitate the design of future-state (TO-BE) processes, challenging
  • existing practices and reengineering workflows to enhance efficiency,

compliance, standardisation, scalability and user experience.

  • Translate approved business processes into clear business and functional
  • requirements, including business requirement documents, user stories, use

cases, acceptance criteria and traceability documentation.

  • Identify opportunities for digitisation, automation, system integration
  • and enhanced data utilisation, ensuring process optimisation and operating

model clarity before technology implementation.

  • Establish and manage project governance structures, including Working
  • Groups, Project Committees and Steering Committees, with clear decision-making

processes, escalation pathways and meeting cadences.

  • Prepare project documentation including agendas, status reports,
  • decision papers and meeting minutes, while maintaining risk, issue, dependency,

assumption, action, decision and change logs.

  • Provide accurate and consolidated reporting on project progress,
  • milestones, risks, resource constraints, dependencies, key decisions and

expected benefits across the assigned portfolio.

  • Ensure adherence to Transformation Office methodologies, standards,

templates, stage-gate requirements and quality assurance processes.

  • Identify key stakeholders and impacted user groups, developing
  • communication, engagement, training and change management plans appropriate to

the scale and impact of each initiative.

  • Build alignment and collaboration among sponsors, business leaders,
  • operational teams, clinical teams, corporate functions and enabling teams,

influencing outcomes without direct reporting authority.

  • Support project owners in defining revised roles, responsibilities,
  • standard operating procedures (SOPs), controls and operational ownership to

facilitate successful implementation.

Monitor user adoption, gather feedback and assess operational performance following implementation, addressing resistance and supporting the embedding of new ways of working.

Proactively identify and manage delivery, operational, clinical, regulatory, information security, data and change-related risks, ensuring appropriate mitigation actions and escalation where necessary.

Coordinate and monitor vendor and consultant performance against agreed deliverables, timelines, quality standards, testing requirements and issue-resolution commitments.

Collaborate with business owners to define baseline measures, target KPIs and expected benefits, tracking outcomes and recommending corrective actions when required.

Capture lessons learned, formally close projects and continuously improve transformation methodologies, process libraries, templates and playbooks to strengthen future project delivery.

What are we looking for?

  • Bachelor’s degree in Engineering, Business Administration, Project

Management, Information Systems, Operations Management or a related field.

Master’s degree in a related discipline would be an advantage.

  • Professional certifications such as PMP, PRINCE2, Agile, Lean Six Sigma,

Business Analysis or Process Improvement are desirable.

  • 5–8 years of experience managing end-to-end transformation,

digitalisation, operational improvement or complex cross-functional projects.

  • Proven experience in process mapping and reengineering, business
  • requirements gathering, project governance, vendor management, implementation

and benefits realisation.

  • Experience within the healthcare sector and exposure to multi-site,

multi-country or matrix organisations would be an advantage.

  • Demonstrated experience coordinating cross-functional teams and

stakeholders to successfully deliver strategic business initiatives.

  • Strong knowledge of project management methodologies, including

Waterfall, Agile and hybrid delivery approaches.

  • Proficient in process analysis and redesign methodologies, including
  • AS-IS and TO-BE process mapping, BPMN, Lean principles and root-cause analysis

techniques.

  • Sound understanding of business and functional requirements
  • documentation, user stories, use cases, acceptance criteria, requirements

traceability and user acceptance testing (UAT).

  • Experience in project governance, including project charters, RACI
  • matrices, project schedules, RAID management, change control, budget tracking

and executive reporting.

  • Knowledge of stakeholder management, organisational change management,

communication planning, training coordination and adoption strategies.

  • Ability to define and monitor KPIs, track benefits realisation and

conduct post-implementation reviews.

  • Strong proficiency in Microsoft Project, Microsoft Planner, Excel,
  • PowerPoint, Visio, Lucidchart, Miro and collaboration/document management

platforms.

  • Understanding of healthcare operations, digital systems, data management,
  • integration principles, information security and regulatory requirements would

be an advantage.

  • Strong sense of ownership, accountability and commitment to delivering

results.

  • Analytical, structured and detail-oriented, with the ability to maintain

a strategic and business-focused perspective.

  • Ability to challenge existing practices constructively and translate

ambiguity into practical and actionable plans.

  • Excellent communication, facilitation, presentation, minute-taking and

executive-reporting skills.

  • Strong stakeholder management and influencing skills, with the ability to

work effectively across functions without direct authority.

  • Proactive, adaptable and resilient, with strong problem-solving

capabilities and the ability to thrive in a fast-paced, evolving environment.

Demonstrates professionalism, credibility and sound judgement when engaging with senior leaders and diverse stakeholder groups.
