Job Description

Responsibilities:

  • Conduct community outreach programs to promote health awareness and preventive healthcare practices.
  • Educate individuals, families, and community groups on hygiene, nutrition, maternal and child health, immunization, family planning, and healthy lifestyle practices.
  • Identify community health needs and provide appropriate health education, guidance, and referrals.
  • Organize and participate in medical outreaches, health campaigns, wellness screenings, and vaccination programs.
  • Perform basic health assessments, including blood pressure, blood glucose, temperature, weight, and other routine health checks where applicable.
  • Support clients in accessing healthcare services by coordinating referrals and follow-up care.
  • Conduct home visits to monitor clients' health status and reinforce health education where necessary.
  • Maintain accurate records of patient interactions, outreach activities, referrals, and community health reports.
  • Collaborate with healthcare professionals and other stakeholders to ensure continuity and quality of care.
  • Encourage adherence to prescribed treatments and promote positive health behaviors.
  • Identify and promptly report disease outbreaks, public health concerns, and emergencies to the appropriate authorities.
  • Participate in health education seminars, training sessions, and continuous professional development activities.
  • Ensure confidentiality and privacy of patient and community information at all times.
  • Comply with organizational policies, infection prevention protocols, and health and safety regulations.
  • Perform other related duties as assigned by management.


Requirements

  • Minimum of an OND in Community Health, Public Health, Nursing, Health Education, Environmental Health, or a related field.
  • Minimum of 1 year of relevant experience in community health, primary healthcare, public health, or wellness programs.
  • Valid professional certification or license (where applicable) is an added advantage.
  • Sound knowledge of health education, disease prevention, and community engagement strategies.
  • Good understanding of primary healthcare principles and public health practices.
  • Excellent communication, interpersonal, and counselling skills.
  • Strong organizational, documentation, and reporting skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Proficiency in the use of Microsoft Office applications is an added advantage.
  • Ability to communicate effectively in English and local languages is an added advantage.


Remuneration: NGN 80,000 per month
