Job Description
Responsibilities:
- Conduct community outreach programs to promote health awareness and preventive healthcare practices.
- Educate individuals, families, and community groups on hygiene, nutrition, maternal and child health, immunization, family planning, and healthy lifestyle practices.
- Identify community health needs and provide appropriate health education, guidance, and referrals.
- Organize and participate in medical outreaches, health campaigns, wellness screenings, and vaccination programs.
- Perform basic health assessments, including blood pressure, blood glucose, temperature, weight, and other routine health checks where applicable.
- Support clients in accessing healthcare services by coordinating referrals and follow-up care.
- Conduct home visits to monitor clients' health status and reinforce health education where necessary.
- Maintain accurate records of patient interactions, outreach activities, referrals, and community health reports.
- Collaborate with healthcare professionals and other stakeholders to ensure continuity and quality of care.
- Encourage adherence to prescribed treatments and promote positive health behaviors.
- Identify and promptly report disease outbreaks, public health concerns, and emergencies to the appropriate authorities.
- Participate in health education seminars, training sessions, and continuous professional development activities.
- Ensure confidentiality and privacy of patient and community information at all times.
- Comply with organizational policies, infection prevention protocols, and health and safety regulations.
- Perform other related duties as assigned by management.
Requirements
- Minimum of an OND in Community Health, Public Health, Nursing, Health Education, Environmental Health, or a related field.
- Minimum of 1 year of relevant experience in community health, primary healthcare, public health, or wellness programs.
- Valid professional certification or license (where applicable) is an added advantage.
- Sound knowledge of health education, disease prevention, and community engagement strategies.
- Good understanding of primary healthcare principles and public health practices.
- Excellent communication, interpersonal, and counselling skills.
- Strong organizational, documentation, and reporting skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Proficiency in the use of Microsoft Office applications is an added advantage.
- Ability to communicate effectively in English and local languages is an added advantage.
Remuneration: NGN 80,000 per month
