This position works within the Community Living Center (CLC) at Black Hills VHA and will have social work responsibilities within the CLC and the facility Palliative & Hospice Care programs.

The responsibilities include but are not limited to participating in IDT and rounding meetings, providing clarification for family members/others, hospital staff, nursing home, etc. concerning Hospice assessment results and care planning which may include in-home or in-facility hospice services. Social worker must be comfortable with end of life conversations and planning, to include Advance Directive and Goals of Care conversations. Expected duties and responsibilities to include but not be limited to the following. The incumbent ... 1. Will use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans and their families/significant others. 2. Conduct psychosocial assessments to determine needs of veterans and family members. 3. Is an active participant in the treatment planning process with other disciplines. 4. Coordinate and refer care with other disciplines and will be responsible for ensuring that the treatment plan is executed in a manner that is timely and appropriate. 5. Provide consultation/education to Veterans and families on community resources, advance directives and VA benefits. 6. Will act as a consultant to other team/staff members. 7. Provide consultation/education to medical residents on social work practice and VA policy and procedure. 8. Will act as liaison between VA and community resources.to include marketing and public relation duties as needed. 9. Provide a full range of social work services within commonly accepted standards of social work practice which includes casework, individual, family and group counseling, discharge planning, case management and other services as appropriate throughout the continuum of care. 10. Will address Advance Directives, Goals of Care, and Organ Donation conversations and related activities in accordance with the standards of their work area. 11. Provide wellness/prevention education and facilitate i:atienVfamily support groups as appropriate. 12. Will, as appropriate, supervise the practice of social work students, or a less experienced social worker. 13. Coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans. 14. Assist the Veteran and family in understanding contributing factors to problems, discuss solutions, and encourage lasting changes to reduce stressors. 15. Participate in committee assignments, as deemed necessary by the service manager. 16. Maintain data and statistical compilations to comply with JCAHO, VA and medical center policy and procedures. 17. Assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. 18. Assess and manage for suicide risk, patient safety and risk, especially as it relates to vulnerable adults. 19. Document social work interventions and activities inpatient's electronic clinical record and ensure appropriate hand off when transferring of patient care. The incumbent demonstrates the capacity to expand clinical knowledge in the profession, provide consultation and guidance to colleagues and stakeholders, and model effective social work practice skills. Apart from providing consultation and case management on difficult and complex clients, the incumbent consults with medical providers and team members on achieving positive outcomes for the Veteran and the facility. Additionally, the incumbent may represent social work and social service issues in care line, facility or community committees and workgroups. Work Schedule: Monday-Friday 7:30am-4pm Recruitment/Relocation Incentives (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
