Position is eligible for the Education Debt Reduction Program (EDRP) a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
General Description of Assigned Duties Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibilities for the coordination of care with a focus on patient education, self-management, and customer satisfaction throughout the continuum of care. By following procedures per established policies and guidelines, influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program-level quality improvement process and initiatives, as well as customer service programs. Functions or Scope of Assigned Duties The Dialysis Vascular and Peritoneal Access Coordinator (VAC) Registered Nurse (RN) is responsible for providing evidence-based care to all veterans with, or approaching, ESRD within our catchment area. The Vascular Access Coordinator assists veterans, family members, and caregivers in receiving the most appropriate options and services to meet their dialysis access and/or peritoneal dialysis (PD) access care needs. Coordinates care with multiple providers across all levels and sites of care. Coordination also includes ensuring services interface within the Veterans Affairs (VA) system and community programs to provide continuity of care to patients. Address psychosocial, nursing, and medical needs of patients and their families/caregivers through participation with the interdisciplinary care team. Coordinates and expedites care from diagnosis to treatment. Analyzes and manages laboratory and radiology results and provides information to providers. Utilizes databases to provide population management and to track patient care through all episodes of care. Evaluates care and outcomes to ensure timely and appropriate provision of services. Advocates for holistic care through patient and family education and provider interaction, and implements an educational plan to meet patient care needs. Manage and coordinate dialysis vascular and PD access placement and tracking for all veterans with or approaching End Stage Renal Disease within our catchment area. Support renal, self-care, home and in-center dialysis clinics with ongoing patient and staff education . Maintain formal reports and/or present as necessary the Quality Assurance and Performance Improvement data on vascular and PD access and other PI data as directed both locally and nationally as required by VA mandate and regional network. Monitor the current vascular and PD accesses and proactively recommend early intervention Attend additional clinics as needed ( surgical and vascular) to facilitate coordinaton of care and provide patient education.
