ABOUT THE ROLE
This is an exciting opportunity to join a passionate and experienced team delivering specialist dementia services that make a genuine difference in the lives of people living with dementia and their carers.
As our Program Coordinator, you'll be the first point of contact for many people accessing Caladenia's services. You'll guide clients and their families through the entire client journey, from initial enquiry and assessment through to service commencement, ongoing reviews and referrals, ensuring each person receives the right support at the right time.
Working closely with clients, carers, families and health professionals, you'll coordinate individual support, develop person-centred care plans and help ensure our services remain responsive to changing needs. You'll also contribute to the coordination of our dementia-specific programs, support carers and participate in service development and continuous improvement initiatives.

ABOUT YOU
We're looking for someone who is organised and passionate about supporting people living with dementia.
You'll enjoy building relationships with clients, carers and colleagues, thrive in a collaborative environment and be confident balancing client coordination with program support.
Ideally, you'll have:
- Qualifications in Nursing, Social Work, Occupational Therapy, Psychology, Community Services or a related discipline.
- Experience working with people living with dementia, older people or family carers.
- Experience in client assessment, case management, service coordination or community services.
- Excellent communication and interpersonal skills.
- Strong organisational and problem-solving abilities.
- A commitment to person-centred practice and continuous improvement.
Experience working within CHSP, Support at Home or My Aged Care is highly desirable but not essential.
