Maintain connectivity with case managers to assure follow-up plans are carried out. Utilize automated healthcare systems to input and monitor patient data. Document all interactions, appropriate issues pertinent to the soldier's case as directed by the Behavioral Health Provider. Maintain ongoing comprehensive supportive care with identified high-risk soldiers during treatment.
