The Registered Nurse (RN) - Mental Health Intensive Case Management-Psychosocial Rehabilitation and Recovery Center (MHICM-PRRC) is responsible for clinically providing outpatient, community-based mental health (MH) services for a subgroup of Veterans with serious mental illness (SMI); including but not limited to transportation, medication management, and in home/community consultation and responds to psychiatric and medical emergencies as directed. The incumbent of this position must have a valid, state-issued driver's license. The Registered Nurse (RN) - Mental Health Intensive Case Management - Psychosocial Rehabilitation and Recovery Center (MHICM-PRRC) is responsible for providing competent, evidence-based care to patients. The person in this role will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization. The MHICM-PRRC program provides clinical community-based case management services to Veterans with serious mental illness (SMI), severe functional impairment, and high Inpatient MH unit utilization, in coordination with existing community and Veterans Affairs (VA) services. The RN - MHICM-PRRC is also responsible for the following duties: Providing outpatient, community-based MH services for a subgroup of Veterans with PMI; including but not limited to transportation, medication management, in home/community consultation. Responding to psychiatric and medical emergencies, as directed. Documenting nursing triage assessments, risk screens, clinical reminders, and patient plan of care in accordance with facility policy. Maintaining professional boundaries to protect patient vulnerabilities and act in the best interest of the patient. Collaborating and supporting MH nursing, along with other disciplines. Demonstrating the ability to cope with and manage competing priorities, demonstrating proficient practice and decision-making, deliberate planning, and critical thinking skills. Holding therapeutic groups at the VA, outings in the community, treatment plans and meets with Veterans one to one to develop goals. Other duties, as assigned. Must pass pre-employment physical examination. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
