Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Oversees all transition and coordination of care activities for post 9/11 service members and Veterans treated at the facility. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Executes position responsibilities that demonstrate expert leadership, experience, and creative approaches to the management program aspects. Demonstrates performance and leadership that is broad enough to impact the standard of care and safety of staff while caring for all patients throughout the continuum of care. Ensures that severely injured and/or ill post 9/11 service members/Veterans with identified case management needs are assigned to a case manager and collaborates with the interdisciplinary team to include but not limited to, Visual Impairment Services Team (VIST) Coordinator, Spinal Cord Injury (SCI) Coordinator, Polytrauma Case Manager, and Women Veterans Program Manager (WVPM). Serves as the facility point of contact for all inquiries regarding transition of post 9/11 service members/Veterans and for those seeking health care from the facility. Addresses Congressional inquiries and Veterans Integrated Service Network (VISN) and Veterans Affairs Central Office (VACO) action items as requested. Coordinates comprehensive program elements and services for preventive mental and physical health evaluations for program participants. Promotes and coordinates evaluations for the Post M2VA Program anywhere within the facility and any outlying Community-based Outpatient Clinics (CBOCs). Prepares and presents program reports. Responsible for referrals between the VA, Military Treatment Facility, and/or community hospitals that promote appropriate and timely inter-facility transfers. Ensures registration of appropriate Veterans for clinical, administrative, and outcome purposes. Establishes performance and outcome standards for the facility M2VA Program that promote quality and efficiency of services in coordination with the overall goals of the facility, VISN, and VACO, as well as evaluate and document the program's effectiveness. Researches community resources that are available to provide continuity of care and to enhance the quality of life of the Veteran. Identifies unmet needs of the post 9/11 population and works collaboratively with facility, VISN, and VACO staff to develop and implement solutions. Develops a data-driven continuous quality improvement program with established goals and outcomes. Develops and implements a plan for outreach to extend services to post 9/11 service members/Veterans who may benefit from care, but who do not presently use VA for their health care needs. Manages the daily operation of the facility M2VA Program and provides accountability for program effectiveness and modification of service patterns to enhance customer satisfaction.
