Applications will be accepted on an ongoing basis and highly qualified candidates will be considered as vacancies become available. The first group of eligible applicants will be referred to the hiring manager on 08/24/2026, and weekly thereafter. Applications will remain on file until 08/17/2027. The Caregiver Support Program includes the Program of Comprehensive Assistance for Family Caregivers (PCAFC) and the Program of General Caregiver Support Services (PGCSS) with in the VHA. Major duties and responsibilities include, but are not limited to, the following: Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient and caregiver education, self-management, and customer satisfaction throughout the continuum of care. Provides oversight of service delivery and coordination of care activities for family caregivers who provide personal care services to eligible Veterans regardless of period of service. Coordinates program implementation with multiple clinical services across all campus locations. Influences care outcomes by collaborating with members of the interdisciplinary team. Provides peers with informal constructive feedback for improvement. Establishes and maintains an ongoing education program for Veterans, caregivers, community agencies, students, and staff to facilitate understanding of clinical practice guidelines, mental health issues, readjustment issues and psychosocial issues facing caregivers. Develops and facilitates CSP focused direction, orientation, coaching, inservice training, staff development, and continuing education programs for Veterans Affairs (VA) facility staff and the community. Develops and implements a plan for outreach to Veterans and Veteran caregivers who may benefit from the CSP program services (but who do not presently use VA for their health care needs) to include outreach to current PGCSS and PCAFC participants to inform them of available services. Maintains a current network of referral resources to include peer support mentoring groups, caregiver skill building and self-care educational opportunities, mental health support, legal and financial services. Serves as a consultant to other VA professionals and administrators on program specific activities. Responsible for monitoring performance measures at the local, VISN, and VACO levels; providing responses to congressional inquiries; and briefings to providers, VA leadership, community organizations, and other internal and external stakeholders.
