This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
The RN's functional role includes following the standardized care coordination model, incorporating case management principles, if applicable, knowledge of processes related to referral and authorization and secure medical documentation exchange. The ability to identify Veterans requiring disease management and the process to obtain these resources internal or external to the VA if applicable. The RN will focus on providing care that is unique to the Veterans needs and continues to monitor, track and intervene, if necessary, in the episode of care across the continuum. The RN will demonstrate effective communication skills to collaborate and co-manage the Veterans care with the multidisciplinary team in a cost-effective manner. Major duties include, but are not limited to: This position potentially requires flexibility in schedule and assignments. Incumbent is an integral member of the Primary Care- Mental Health Integration (PC-MHI) team where the majority (e.g., 75%) of the workload occurs. The nurse also fulfills a key role within the Mental Health Continuum of care by providing Mental Health Treatment Coordination (MHTC) services (25% of the time) as part of the Behavioral Heath Interdisciplinary Program (BHIP) team at the Community Based Outreach Clinic (CBOC). Utilizes appropriate resources and tools to document, track, and assure follow through on patients with high-risk mental health needs or care coordination. Provides collaborative care management, within the PCMHI program, for Veterans whose mental health problems are being managed by their PC provider. Conducts a comprehensive assessment of the patient's health and psychosocial needs, including health literacy status and deficits, and develops a plan collaboratively with the patient, other health care providers, community and family or caregiver to maximize health care responses, quality, and cost-effective outcomes. Completes structured clinical assessments for participants. This requires knowledge of medical terminology, diagnostic categories, and chart review. Provides individual psychoeducation and direct advice regarding the management of mental health and substance abuse symptoms. Interacts directly with patients and primary care providers facilitating ongoing evaluation, treatment planning, and communication allowing evaluation/care to remain in primary care.
