This Patient Aligned Care Team (PACT) Registered Nurse Care Manager in Alexandria, MN is a GAP RN that provides a significant portion of clinical support across Alexandria, Brainerd, and Montevideo Community Based Outreach Clinics (CBOCs). This position prevents service interruptions created by planned and unplanned leave at the CBOCs through a wide range of nursing modalities.
Major Duties include but are not limited to: Assisting with providing coverage between the Alexandria, Brainerd, and Montevideo Community Based Outreach Clinics (CBOCs). Communicates and collaborates as an effective member of the interprofessional team, demonstrating leadership responsibilities such as team leader/charge nurse. Shares knowledge and learning opportunities that enhance current nursing practice and self-evaluation for self and others on the team. Delivers self-directed, professional nursing practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) for complex patients based on conscious and deliberate planning with occasional supervision. Self-directed in providing primary care coordination for the Patient Aligned Care Team (PACT), overseeing transitions of care, developing and implementing Veteran-centered nursing care, delivering preventive and chronic disease management interventions, and providing care through multiple modalities, including face-to-face visits, virtual encounters, secure messaging, and telephone based services with occasional supervision. Identifies and manages assigned patients requiring RN care through daily huddles and team collaboration; develops individualized care, provides health coaching and motivational interviewing, and delivers ongoing management for patients with chronic and ambulatory care-sensitive conditions. Uses data-driven risk indicators to identify rising- and high-risk patients, prioritize focused panel management, and coordinate appropriate referrals for enhanced care management. Applies Whole Health principles and appropriate task delegation to strengthen team functioning, enhance care coordination, and meet PACT performance goals.
