This job announcement is seeking a Community Care (CC) Coordinator Registered Nurse {RN) that falls within Chief of Staff at the HJ Heinz Campus in Pittsburgh, Pennsylvania. The CC Coordinator serves as a liaison to both internal and community providers and is responsible for managing the care of the Veteran throughout the consult process.
The Community Care Coordinator RN supports the medical center's mission to improve timely access to care, ensure Veterans who want to receive care in the community are referred and scheduled into the community, and support the delivery of high quality, timely care to the Veteran receiving community services. The CC Coordinator will work collaboratively within the Office of Integrated Veteran Care {IVC) to triage Request for Services {RFS) to coordinate care processes defined by the IVC Field Guidebook to include developing a care coordination plan, reviewing medical documentation from community providers, communication of care coordination needs, and supporting the veteran through the completion of the episode of care. The CC Coordinator RN will influence care outcomes by collaborating with members of the interdisciplinary team both within the Veterans Affairs (VA) and in the community. Duties include but are not limited to the following: Leads the provision of nursing care to patients with complex care needs utilizing the nursing process in alignment with the ANA Standards of Nursing Practice. Applies clinical knowledge/judgment to lead staff involvement in planning, decisionmaking, and evaluating outcomes within the immediate practice setting. Effectively functions as a leader, with demonstrated outcomes, for the immediate practice setting. Improves Veteran/patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting. Applies patient, family, and caregiver knowledge, values, and beliefs into care delivery without judgement or discrimination. Serves as an advocate for patients, families, and caregivers, improving outcomes for the immediate practice setting. Serves as a resource for patients, caregivers, and staff to address and resolve ethical issues in the immediate practice setting. Leads the implementation of high reliability principles and I CARE values to increase consistency of care delivered and improve patient outcomes. Preferred Experience: BSN 5 Years, Case Management and Utilization review, and computer knowledge.
