The Case Management Urology Registered Nurse (RN) delivers competent, evidence-based care to assigned patients. Responsibilities include patient management, care coordination, and discharge planning for both inpatient and outpatient settings. The RN assists Veterans, families, and caregivers in accessing the most suitable options and services for their complex healthcare needs, addressing conditions ranging from acute and chronic to multiple, complex, catastrophic, or life-threatening illnesses. The Case Management Urology RN will assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. This RN will influence care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Major Duties include but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Providing patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Coordinating care with multiple providers across all levels and sites of care. Addressing psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. Evaluating care and outcomes to ensure timely and appropriate provision of services. Demonstrating leadership in delivering and improving holistic care through collaborative strategies with others. Providing peers with informal constructive feedback for improvement. Fostering a safe and supportive environment conducive to the professional development of healthcare professionals. Supporting colleagues through CM knowledge sharing to provide safe, quality nursing care. Sharing educational findings, experiences, and ideas with peers. Questioning clinical practices for the purpose of providing evidence-based care.
