The Northeast Ohio VA Healthcare System is recruiting for a Registered Nurse (Caregiver Support Program) within the Social Work Service. The Registered Nurse (Caregiver Support Program) blends clinical expertise with technology to improve how Veterans receive care. In this role, you will support safe, high-quality, date-driven practice by enhancing clinical systems, advancing workflow efficiency, and strengthening the connection between staff, information, and patient outcomes.
The duties of a Registered Nurse (Caregiver Support Program) include, but are not limited to: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex patient care. Provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, and health counseling. Possesses thorough knowledge of PCAFC [Program of Comprehensive Assistance for Family Caregivers], PGCSS [Program of General Caregiver Support Services], clinical documentation requirements, current standards of care, and all regulatory compliance requirements and guidelines. Accountable for utilizing critical thinking skills and sound judgment to ensure Veterans and caregivers are provided with thorough, comprehensive care options within the scope of the CSP [Caregiver Support Program]. Provides assessments for PCAFC applicants at Veterans Affairs {VA) facilities, via phone and telehealth, as well conducts home visits to caregivers and Veterans approved in the program. Provides referrals and/or consults to VA and community based social services. Collaborates with the interdisciplinary team for case review with high inter-rater reliability, efficiency, and objectivity as top priorities. Demonstrates knowledge and ability to apply principles of the CSP. Applies the nursing process and care coordination principles within a collaborative, interdisciplinary practice setting, which will then enable effective application of CSP practices and principles. Provides a thorough assessment of patient care needs to determine the most appropriate level of program participation. Uses knowledge of the CSP to matrix Veteran level of functioning with the appropriate CSP services (i.e., PGCSS; PCAFC; and/or Veteran entitlements and benefits) to ensure patient safety, recovery, and program stewardship. Evaluates, systematically, current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Coordinates within the CSP to develop and implement strategies for performance improvement and enhancement of the program.
