Diagnose and manage acute and chronic medical conditions specific to older adults. Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams. Review, reconcile, and optimize medication regimens to minimize polypharmacy and adverse drug events. Provide consultative or primary outpatient, inpatient, and/or long-term care to geriatric patients. Assess and manage neurocognitive disorders, including dementia, delirium, and depression. Coordinate patient care with other healthcare providers, specialists, therapists, pharmacists, and social workers. Facilitate referrals to specialty services, community agencies, rehabilitation programs, and support resources.
