Coordinate patient care and services in a clinic setting through face-to-face encounters or via telecommunication technology. Screen and prioritizes patient's condition for urgent and emergent care. Identify expected outcomes in a plan of care individualized for a specific patient, group, or population. Provide health education/coaching to patients, significant others, and/or family members/caregivers.
