The Telehealth Licensed Practical Nurse ( LPN) will provide clinical and technical support to patients receiving care via telemedicine, teleconsultation, tele-education services. This position is located with Telehealth Service department in Sierra Vista CBOC. Primary purpose of the LPN will perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran.
Duties include but are not limited to: The LPN is responsible and accountable for individual nursing practice. Independently initiates, performs, and completes assigned patient care duties with technical proficiency across variable patient populations. Uses sound clinical judgment to select the appropriate order and sequence of procedures and treatments based on patient condition and care priorities. Accurately recognizes, interprets, reports, and documents relevant patient information with minimal need for review. Adjusts care delivery proactively in response to changes in patient condition, practicing safely and consistently within established policies and procedures. The Telehealth LPN performs telehealth duties with substantial independence and demonstrates consistent, reliable management of a wide range of telehealth operations. The position is typically stationed at a clinical care delivery site (VA Medical Center, CBOC, Rural, etc.), with duties that may require travel. Responsibilities include serving as a skilled Tele-Presenter, performing imaging and advanced clinical data support for SFT applications, managing real-time clinical telehealth events, and providing patient education confidently and accurately. Additional responsibilities include coordinating both simple and complex intra- and inter-facility telehealth appointments, offering comprehensive technical support, and performing installation, configuration, and basic maintenance of telehealth technologies. The LPN may contribute to evaluating telehealth workflows, support performance improvement initiatives, and could be certified to function as a preceptor in Clinical Video Telehealth (CVT) and/or SFT operations. All activities are performed in alignment with VHA, VISN, and facility policies and support broader telehealth program goals. Also: - Demonstrates technical proficiency and independently organizes and completes patient care activities for varied populations. - Applies sound judgment in selecting procedures, sequencing tasks, and recognizing clinically significant findings. - Identifies patient care needs, including psychosocial and age-related considerations, and responds appropriately. - Safely administers all authorized medications and treatments, evaluating and documenting patient response and reporting deviations promptly. - Recognizes emergent situations, initiating appropriate interventions and coordinating immediate assistance. - Applies advanced understanding of patient behavior to foster therapeutic relationships and promote positive engagement. - Protects patient rights, privacy, and dignity in all interactions and exhibits strong customer service. - Performs and supports complex diagnostic procedures, including preparation, equipment handling, and monitoring pre-, intra-, and post-procedure. - Serves as a preceptor to orient, train, and mentor less experienced LPNs/LVNs and support staff. - Actively pursues continuing education and promotes ongoing improvements in team practice. Preferred Experience: Prior experience in Ambulatory Care setting and/or experience with virtual care Work Schedule: Full-Time; day shift, 0730-1600. Depending on the needs of the facility.
